What Lube Ingredients Actually Mean for Sensitive Skin (and What the WHO Recommends)

If a lubricant has ever left you stinging, dry the next day, or generally irritated, the ingredient list is usually where the answer lives, but most labels don't tell you the two numbers that actually matter. The World Health Organization publishes clear thresholds for what makes a lubricant safe for the tissue it touches. Here's what those thresholds are, what the published research says about how many commercial lubricants actually meet them, and how to use that information without needing a lab.

The two properties that actually matter: osmolality and pH

Osmolality is a measure of how concentrated a solution is, and it matters because tissue inside the body naturally sits at a specific osmolality. A lubricant that's far more concentrated than that tissue will draw water out of the surrounding cells, which is what causes the drying, stinging or damage some people notice. The World Health Organization states directly that "the osmolality of a personal lubricant should not exceed 380 mOsm/Kg to minimize any risk of epithelial damage." Separately, in its technical specifications for water-based lubricants intended for bulk procurement, the WHO sets a maximum working limit of 1200 mOsm/kg. Both figures come from the WHO: 380 is the ideal target for minimising tissue damage risk, and 1200 is the outer ceiling used for large-scale procurement specifications. Many lubricants on general retail shelves sit well above both.

pH matters for a different reason: vaginal tissue in particular maintains a specific, mildly acidic pH range as part of its natural defence against infection. A lubricant with a pH far outside that range can disrupt that balance, at least temporarily.

Neither property is listed on most commercial lube packaging, which is exactly why this is worth understanding rather than assuming a "sensitive skin" or "natural" label has already handled it for you.

What the actual research found

A peer-reviewed study published via the National Institutes of Health (hosted on PMC, the NIH's own repository), which directly tested a range of commercially available vaginal lubricants, found that most of the products tested had pH and/or osmolality values outside the ranges recommended by the WHO. The same study also reported cytotoxicity, meaning some level of cell damage, in laboratory testing of certain products.

That's not a reason to assume every lubricant is harmful. It is a reason not to assume a product is automatically fine for sensitive skin just because of how it's marketed. The gap between "recommended by WHO" and "what's actually on shelves" is real and has been measured directly, not just claimed.

Why this shows up as irritation for some people and not others

Everyone's tissue tolerance is different, which is why the same lubricant can feel fine for one person and cause stinging for another. Higher osmolality and pH imbalance don't necessarily cause an obvious reaction every time, but they increase the theoretical risk of irritation and, per the WHO's own language, epithelial damage with repeated use. If you've noticed a pattern of irritation with a specific product, that's a reasonable thing to bring up with a pharmacist or GP, particularly if it persists or recurs.

What to actually look for

Since osmolality and pH aren't printed on most labels, there are a few practical things you can do instead:

Look for water-based or silicone-based formulas over glycerin-heavy ones. Glycerin and similar humectant ingredients pull water osmotically, which is part of the mechanism behind higher osmolality readings in some products. This doesn't mean every glycerin-containing lube is a problem, but it's a reasonable starting point if you're specifically trying to reduce osmotic irritation. Fewer, simpler ingredients are easier to reason about. A shorter ingredient list makes it easier to notice if a specific addition (a fragrance, a warming or cooling agent, a preservative) coincides with irritation, simply because there's less to isolate. If a product states it's tested to WHO-aligned osmolality and pH ranges, that's a meaningful claim, not just marketing language, given how directly the WHO has published its own thresholds and given that peer-reviewed testing has shown many products don't meet them. We can't verify every individual product against a lab report, so treat ingredient lists (glycerin, paraben and fragrance-free) as the practical proxy rather than assuming any specific product meets the WHO figures exactly.

The condom compatibility point that matters regardless

Separate from the sensitivity discussion, one rule applies to every latex condom regardless of how gentle a lubricant is on skin: oil-based lubricants and oil-based products (including many massage oils, petroleum jelly and some balms) degrade latex and can cause a condom to fail. If you're using a latex condom, stick to a water-based or silicone-based lubricant. This isn't related to sensitivity at all, it's a material compatibility issue, and it applies even to lubricants that are otherwise gentle.
